The Transition Age Youth (TAY) program in Contra Costa County, California provides housing referrals and support services for young adults with serious mental health issues:
- Ages: 16–25 years old
- Services: Intensive support services and housing referrals
- Goal: Help young adults achieve recovery-focused goals and function independently in their homes and communities
- Partners: Fred Finch Youth Center, Contra Costa Youth Continuum of Services, Greater Richmond Interfaith Program, The Latina Center, and Contra Costa County Mental Health Services
