For youth navigating the hardest transitions —
structure, support, and a team that stays.
G.L.O.M. TAY is a comprehensive behavioral health and life transition program serving youth and young adults ages 16–25. We operate as a four-tier continuum of care — clinical, psychiatric, residential, and life transition services — coordinated under one provider, one team, and one consistent relationship.
Our Mission | To empower youth and young adults in transition by providing the tools, guidance, and support necessary to create strong, independent futures.
Support that Empowers
More than services — we provide structured, consistent care that meets each young person where they are. From clinical treatment to daily life skills, every support is designed to build real capacity for independence, not just short-term stability.
Connections
that Matter
Isolation is one of the biggest barriers TAY youth face. We build intentional relationships — with peers, mentors, care teams, and community — so that no young person is navigating their transition alone.
Opportunities
for Success
We believe the path forward looks different for every person. Through education linkage, vocational training, employment readiness, and housing support, we open doors and help youth and young adults walk through them with confidence.
Youth and young adults who deserve more than a referral and a waiting list.
G.L.O.M. TAY was built for youth and young adults navigating some of the hardest transitions life can bring — and who deserve real, consistent support while they do it.
The need is real. So is our response.
of TAY have a mental health diagnosis
youth aging out of foster care develop substance dependence
of young men who age out are convicted of a crime
of foster youth want to go to college — and need support to get there
One program. Four tiers. No gaps.
Most programs hand youth off as their needs change. We don’t. The same team walks alongside youth and young adults — through every tier, every transition, every step.
