G.L.O.M. TAY (Transitional Age Youth) is a comprehensive behavioral health and life transition program serving youth and young adults ages 16–25. We specialize in supporting young people who are navigating some of the most vulnerable and critical years of their lives — particularly those transitioning out of foster care, exiting probation, experiencing housing instability, or living with untreated mental health and substance use challenges.

The transitional age years — roughly 16 to 25 — are among the highest-risk period in a young person’s life. This is when mental health conditions often first emerge, when the safety nets of childhood systems drop away, and when the absence of consistent support can set the trajectory for years to come. Our program exists because this population deserves more than a referral and a waiting list.

Our program operates as a four-tier continuum of care, meaning we meet young people at whatever stage they’re in and walk alongside them through every transition — without ever changing providers, disrupting relationships, or starting over. The same team. The same commitment. Every step of the way.
