What is a TAY program?
A Transitional Age Youth (TAY) program provides behavioral health support, life skills, housing assistance, and community connection for young people ages 16–25 who are navigating major life transitions — including aging out of foster care, exiting probation, or managing mental health and substance use challenges without consistent support.

Do you provide mental health support?
Yes. Our licensed clinical staff provide individual therapy, group therapy, trauma-informed care, crisis stabilization, and substance use counseling — drawing on ACT, CBT, and other evidence-based therapeutic frameworks tailored to each individual’s needs.
Is psychiatric support available?
Yes. Our integrated psychiatric team provides evaluation, diagnosis, medication management, and ongoing monitoring — coordinated directly with your clinical and case management team so your care stays connected throughout.
Is housing available through the program
Residential services are available only as part of Tier 3 — Crisis Residential & Step-Down — for eligible youth and young adults ages 18–25 who require a structured, supervised living environment as part of their crisis stabilization plan. Residential placement is based on clinical need and availability and is not a standalone housing program.
How are services delivered?
G.L.O.M. TAY’s primary service delivery model is telehealth, making it accessible regardless of transportation or location. In-person services are available based on county, school, or partner need. Contact us to discuss the delivery model that works best for your situation.

How do I apply?
Yes — and it’s one of our core service areas. We provide support with school re-engagement, college navigation, GED pathways, vocational and trade program linkages, job readiness, resume building, interview preparation, employment placement, and ongoing coaching and retention support.
Does it cost anything?
G.L.O.M. TAY services are available through Medi-Cal, Managed Care plans, Collaborative Contracts, and EPSDT. Private pay options are also available. Contact us to discuss your eligibility and next steps.
What happens after I complete the program?
Participants leave with housing resources, employment connections, community linkages, and the life skills to sustain independence. Our goal is not just program completion — it’s long-term stability. Step-down planning begins well before transition and is built collaboratively with the young person and their care team.

How is service delivered?

What does the continuum of care look like?
G.L.O.M. TAY operates across four tiers: Prevention (16–25), Outpatient Treatment & Stabilization (16–25), Crisis Residential & Step-Down (18–25), and Restoration & Independence (16–25). Youth move between tiers based on clinical need — without losing their care team or continuity of services.
Do you coordinate with county systems?
Yes. We work directly with county behavioral health, social services, probation, schools, and hospitals to ensure seamless referral, placement, and ongoing care coordination. Our case managers actively maintain communication with referring partners throughout a young person’s time in the program.
